That is the continue screen from some arcade game I played when I was a kid at a pizza parlor. The guy is getting swallowed buy a giant mouth until you put a quarter in. Please help me figure out this title!

May 27, 2012 - The guy getting swallowed by a giant mouth at 2:04 was a medieval like side-scroller where you could jump on people and crush their heads. This is the continue screen. I remember the start menu was so scary, like a horror movie where some creature's hand had a knife and was gonna stab a girl in the head. But I can't recall the name. Somebody please help! I have looked all over the internet for information on this game and this is the closest I've got. What is the this gamed called! Please!
