How the Body Works : The Pancreas
The Pancreas The pancreas is shown here with the foreground highly magnified to reveal its inner anatomy. Ninety-nine percent of pancreatic tissue is composed of acinar glands, which secrete an alkaline digestive juice into the duodenum via the pancreatic duct to help digest food. The endocrine areas of the pancreas, known as the islets of Langerhans, are composed of two major types of cell. The alpha cells secrete the hormone glucagon, and the beta cells secrete insulin, into the bloodstream. These hormones work in opposition to control the blood glucose level. Insulin promotes the uptake and usage of glucose in the tissues, particularly skeletal muscle and fat, and reduces glucose production in the liver. Glucagon has an anti-insulin effect in the liver, increasing glucose release. The anatomy of the pancreas also includes: the ampulla of the common bile duct: the pancreatic duct, which carries digestive juice containing enzymes to the duodenum; the acini, the exocrine portions of the pancreas which secrete digestive juice into the pancreas; the splenic arteries which conduct oxygenated blood into the pancreas; and the mesenteric veins, which carry deoxygenated blood containing insulin and glucagon away from the pancreas.

Charlotte Rae shares her experience with pancreatic cancer
Chalotte Rae, familiar to millions of TV viewers as the housemother on the '80s sitcom The Facts of Life, was diagnosed in 2009 with pancreatic cancer, an often-silent killer with few if any symptoms until it is too late. Some 40000 new cases of pancreatic cancer are diagnosed each year, and by the time diagnosis is made, up to 80 percent of patients are no longer candidates for treatment. "I had no symptoms," Rae says. "I had absolutely no symptoms. None whatsoever." But Rae was, in a way, lucky. Because she had a family history of the disease - her mother, uncle and older sister all died from the disease - she underwent early screening, which detected the cancer at an early stage. "We're working hard to develop tests for earlier diagnosis," says Howard Reber, MD, director of the UCLA Pancreatic Cancer Program. The goal is to create something similar to the PSA test now done to detect prostate cancer in its early stages, before it has had a chance to spread. "In patients where we know that there's an increased likelihood of the development of the disease, we can screen them, we can get CT scans, we can get endoscopic ultrasounds," Dr. Reber says. In Rae's case, the cancer was detected and found to be contained, but it was growing fast. Surgery was performed to remove the cancer, and now, following surgery and chemotherapy, Rae is cancer free. The Diabetes wiREless Artificial pancreas consortiuM (DREAM) recently evaluated the MD-Logic Artificial Pancreas (MDLAP) system in an innovative trial project taken place in Israel, Slovenia and Germany. The aim of the trial was to provide a real solution to patients with diabetes, using the MDLAP in a diabetes camp outside the confines of the hospital. This is a significant landmark in research: this is to the best of our knowledge the first time in the world that such a trial project has been taken outside the hospital, illustrating a "normal" life for youngsters with diabetes, while using an automatic insulin delivery system during the night. The MD Logic system which was developed at Schneider Children's represents the "artificial pancreas" technological solution comprising of an off-the-shelf subcutaneous glucose sensor that monitors the glucose level and an insulin pump. The sensor and pump are connected to a computer that programs the information and stipulates the amount of insulin that should be released to the body in order to maintain blood glucose balance. This innovation "closes the loop" between the sensor and the pump and relieves the patients with diabetes from the daily burden of dealing with their diabetes and has the potential to significantly improve the quality of life of patients with diabetes.
