Situated in the middle of Southern Africa, Lesotho is known as the Kingdom in the Sky because of its lofty altitude - it has the highest lowest point of any country in the world (1400m) and is the only country to be entirely above 1000m. Taking a short internal flight from Johannesburg to Maseru, Lesotho's capital, Toby drives to the starting point for his adventure - the Malealea Lodge.

This is Lesotho. A small mountainous Kingdom entirely surrounded by South Africa. I spent several years of my childhood here and when I returned as a young man, passionate about mountain biking, I re discovered paradise. Forget fences, forget limits, forget riding as you seen it before. A dream was born. An endurance mountain bike race for only the toughest of riders. 6 days, 500 km, 14 000 m of climbing. I wanted to see riders on paths taken only by herdboys and animals. I wanted to share with others the untamed beauty of this country, it's rugged landscapes and the warmth of its people. An experience which I'm not sure how to put into words. A place which people simply need to come and see for themselves. This is Lesotho Sky. According to the Economist, for example: Every dollar spent promoting breastfeeding in hospitals yields returns of between $5-67. And every dollar spent giving pregnant women extra iron generates between $6-14. Nothing else in development policy has such high returns on investment. The 9km drive up the Sani Pass is a 4x4 adventure drive along a rough road connecting Kwa Zulu Natal with Lesotho in the Drakensburg Mountains. The day we did it the weather was appalling! The reward for our endeavours was a cool beer in the bar of the Highest Pub in Africa, at the strategically placed Sani Top Chalets!!

"The Health Systems Assessment is a very, very important survey for the type of work I do," says Mpopo Tsoele, the Decentralization Advisor for Lesotho Ministry of Health and Social Welfare. "The HSA gives you a full picture of the situation on the ground, which I found very useful as I advised the Minister on how to decentralize primary health care. The HSA clearly gives you a picture of the problems and issues so as you prepare to transfer responsibility from the central government to local authorities, you don't move the problems with you."
