This describes a pathological condition involving the position of the "hallux" in relation to the first metatarsal. A bunion deformity can clinically present with a variety of characteristics. The foot itself may present with a wide splaying of the forefoot and a painful bump on the medial aspect of the first metatarsal phalangeal joint. In addition, the hallux may be abducted from the midline of the body, with a valgus rotation in the frontal plane. A radiographic analysis of a bunion deformity in the Anterior/Posterior or Dorsal/Plantar view will reveal a variety of pathological components. Most notably so, is the exaggerated inter-metatarsal angle between the first and second metatarsal. This may be accompanied by a displacement of the first metatarsal from its position over the sesamoids, such that the metatarsal demonstrates a medial alignment away from the sesamoids which lie to the lateral side. In some cases, the proximal articular set angle at the head of the first metatarsal may be off-set. This "PASA" is one of the factors which determines the position of the proximal phalanx on the metatarsal during movement as well as at rest. UPDATE: I've had the left foot done: www.youtube.com I had bunion surgery in May 2008, so this video shows how I'm doing almost two years later. For those of you who want specifics: it was a Scarf/Akin procedure. I have two screws in my foot and one in my toe. I was awake during the surgery, which lasted one hour. The pain was fairly bad the first day or two but was manageable. I could walk with two crutches after about two weeks and could walk with one crutch after about six weeks. The movement in my big toe joint is fine for walking, running, dancing, etc., and I can wear normal shoes including high heels. PS: the tattoo is from last year!

Probably the same person who named an enlarged thyroid a goiter, but seriously, bunions are the common description of a bump on the side of the big toe. Really they are much more than that. Bunions are known in medical terms as Hallux Abducto Valgus which in English is a crooked big toe joint with a bump on the side. This is actually a progressive misalignment of the big toe joint which starts with a slight leaning of the big toe toward the second and then gradually produces the characteristic bump and joint symptoms. In the late stages of progression, the big toe actually can overlap or underlap the second and cause reactive hammertoes (contracted) of the other toes. Even though they are extremely common, most people wait until they have significant symptoms and deformity before they seek medical attention. Often they have had pain in shoes and with activity for years when they finally come into the office. This is due to the misconception that they always require surgery for treatment.
