Cockpit Voice Recorder - Air Florida Flight 90 Crash
Viewer Discretion Advised Air Florida Flight 90 was an Air Florida flight of a Boeing 737 airliner that crashed into the 14th Street Bridge across the Potomac River in Washington, DC on January 13, 1982 immediately after takeoff in a severe snowstorm. The accident claimed the lives of 78 people, including four motorists on the 14th Street Bridge. However, a few survivors from the shattered aircraft were rescued from the icy river by a combination of heroic efforts of civilians and professionals. Some of that heroism was commended during President Ronald Reagan's State of the Union speech a few days later. The National Transportation Safety Board determined that the cause of the accident was pilot error. The pilots failed to switch on the engines' internal anti-icing equipment, used reverse thrust in a snow storm prior to take-off, and failed to abort takeoff per FAA regulations even after detecting a power problem while taxiing, and visually identifying ice and snow buildup on the wings.

He describes how the box withstands high temperatures and crash velocities because it is made from Inconel: A superalloy steels that is used in furnaces and others extreme environments. The flight data recorder he shows is a Sundstrand FA-542 and was likely used on a DC-9 in the 1970s, although it could have been used as late as 1988 on a Boeing 727.

BOEING 747 CRASH CVR COCKPIT VOICE ROCORDING CFIT BLACK BOX RECORDING
On February 19, 1989, a Boeing 747-249F operating as Flying Tiger Flight 66 was flying an international cargo flight from Singapore to Kuala Lumpur, Malaysia. The aircraft impacted terrain 12 miles from the airport, resulting in 4 fatalities. The aircraft was assigned an non-directional beacon (NDB) approach to Runway 33 at Sultan Abdul Aziz Shah Airport, Kuala Lumpur, after having flown 30 minutes from Singapore Changi Airport.[1] In descent, the flight was cleared to "Kayell" with a morse code of "KL" of which four separate points on the ground were commonly called by Malaysian ATC albeit with different frequencies. Two separate radio beacons were identically coded "KL" as well as the VOR abbreviation (Kuala Lumpur shortened to "KL") and the airport was also sometimes referred to as "KL" by local ATC (instead of the full "Kuala Lumpur"). The crew was unsure to which point they were cleared, and the cockpit voice recorder revealed that the crew argued about which radios should be set to which frequencies and which approach was actually going to be conducted. (Even in the last few moments of the flight, the captain referenced the ILS approach for runway 33 which was notamed as inop on the flight release and the ATIS, additionally the crew was told by ATC that the ILS approach was not available.) ATC radioed to the flight, "Tiger 66, descend two four zero zero [2400 ft]. Cleared for NDB approach runway three three." The captain of Tiger 66, who heard "descend to four zero ...

Lokomotiv Yaroslavl Crash - Audio recording from black box
The recording from the black box onboard of Lokomotiv Jaroslavl plane as they are taking off. Seconds later they crash in to a beacon tower mast located about 450 metres (1480 ft) from the end of Runway. The Lokomotiv Yaroslavl plane crash occurred at 16:05 Moscow Time on Wednesday, 7 September 2011, when a Yak-Service Yakovlev Yak-42, carrying the players and coaching staff of the Lokomotiv Yaroslavl professional ice hockey team, crashed near the Russian city of Yaroslavl. The aircraft ran off the runway before lifting off, failed to gain altitude, struck a tower mast, caught fire and crashed 2 km (1.2 mi) from Tunoshna Airport. Of 45 onboard, 43 died at the scene. One of the two survivors, Alexander Galimov, died five days later in hospital. The crash is under investigation. Lokomotiv Yaroslavl, which was a member of the Kontinental Hockey League (KHL), Russia's top league, was on its way to Minsk, Belarus, in order to start the 2011--12 season.
